Re: Hai joshex!
controversial as in naughty and or politically fucked?
bubble bath babes (NES)
Hot Slots (NES)
Peek-a-boo poker (NES)
Honey Peach (NES)
Zero the Kamikaze Squirrel (SNES)
Dōkyūsei 2 (SNES)
Conker's Bad Fur Day (N64)
BMX XXX (Game Cube)
Doki Doki Majo Saiban/ Shinpan (NDS) [Nintendo's slogan at the time: "Touching is good" litterally they came up with that slogan a week or so after the game was listed as being ported to English]
Fire Emblem: path of radiance (Game Cube) [the whole game suffered at the hands of dicks who sacrificed game-play standards to input a "anti-racism" message]
Violent:
Duke Nukem (gameboy color, N64)
Turok: Dinosaur Hunter
Turok 2: seeds of evil
Turok 3: Oblivion
Turok: Rage Wars
Resident Evil (N64, NDS, Gamecube)
and Conker's Bad Fur Day (N64)
this is just a list of the ones that made it to the western nations.
also can't forget teh Mortal Kombat on NES but nintendo pacified it, they removed a lot of gore and finishers.

Re: Hai joshex!
Mad World and Beyonetta 2 didn't receive much controversy for violence and sexual stuff but should have. It's actually been a point of wonder that Mad World received no attention when it was significantly darker and more violent than other games that got a lot of flack during it's generation, and it was on the Wii which people thought of as targeting younger gamers.
No More Heroes 1&2 on the Wii is pretty up there for both violence and sexual themes (the main goal of the first game is to kill as many of the other top assassins in the world for the expressed goal of possibly having sex with some random girl that knows the rankings).
