If you're carrying drugs, you may get attacked by drug addicts, and the police (in urban areas). If you're carrying medicines and other legal supplies, you may get jumped by thieves and gangsters. (away from the cities)
Both types of trading net you money. Drug dealing gets the most cash, but dealing in legal supplies, not so much. However, the royal guards will leave you alone if you dont deal drugs.
Some sidequests and endings may involve fighting too.
